Personal Data Processing Authorization

As the data subject, I freely and voluntarily grant prior and express authorization to SUPERANDO Y EDUCANDO SAS, Tax ID (NIT): 900.906.806-3, to process my personal information, either directly or through its data processors. I declare that I have been informed about the processing to which the information I provide will be subject, which consists of collecting, storing, using, recording, managing, confirming, verifying, referencing, deleting, and updating the personal information I have provided or that is collected about me.

Likewise, I declare that I have been informed that the purpose of processing my personal data is to allow SUPERANDO Y EDUCANDO SAS-COLEGIO LICEO MODERNO to know and verify the information I provide for the purpose of enrolling my minor child in the school, and for any matters arising from or in connection with such enrollment. SUPERANDO Y EDUCANDO SAS-COLEGIO LICEO MODERNO undertakes to comply with its obligation of secrecy regarding personal data and its duty to treat such data confidentially, and adopts the technical, organizational, and security measures necessary to prevent its alteration, loss, or unauthorized processing or access, in accordance with the Data Protection Law and the international treaties signed by Colombia governing this matter.

In addition, I declare that I understand and have been informed of my rights as the data subject under Law 1581 of 2012:

• a) To access, update, and correct my personal data.
• b) To request proof of the authorization granted.
• c) To be informed of how my personal data has been used.
• d) To file complaints with the Superintendence of Industry and Commerce for any violation in the processing of personal data.
• e) To revoke the authorization and/or request the deletion of data when constitutional and legal principles, rights, and guarantees are not respected.
• f) To access, free of charge, my personal data that has been subject to processing.
• g) To be informed that answering questions about sensitive data is optional.

I declare that the information I have previously provided to SUPERANDO Y EDUCANDO SAS-COLEGIO LICEO MODERNO is complete, accurate, and truthful; accordingly, I authorize it to remain in the files and/or databases of SUPERANDO Y EDUCANDO SAS-COLEGIO LICEO MODERNO for as long as the company exists.
